The objective of this guide is to help our Healthier SG users familiarise with the Health Plan feature
To access the health plan feature:
- Head to the patient’s profile
- Click on ” Health Plan”
At this point, Vanda will check whether the patient is enrolled into healthier SG. Only HSG enrolled patient have access to Health Plan at this time. - Under the Health plan tab, you will be able to see all the past health plans.
- Click on ” + New Health Plan” to start creating a new health plan.
The Health plan will consist of a few segment.
- Patient Information – Do ensure that the patient information are accurate as the data will be sent to Healthier SG repository.
- Clinic Information – Do ensure that all of the Clinic’s Information are up to date, otherwise, you may request for your administrator to update them in Vanda Manager before submitting the health plan
- Specific chronic diseases – Certain chronic diseases will require certain health suggestions to be made to the patient. Vanda will inform you of the recommended fields.
- Before proceeding with health planning, Doctors may view the questionnaire response or copy the last health plan goals if one existed prior.
Crafting the Health Plan
The health plan can consist of the following topics. Doctors may select what topics to be included in the health plan by checking the specific topics:
The folllowing screenshot showcases the Health Goals to be met. A notification informs users of the system selected health goals due to the above selected chronic conditions.
Doctor may choose to recommend vaccination to the patient. Similarly, Doctor may uncheck the topic (highlighted in the image below) to remove it from the health plan
Doctors may plan an exercise regiment for the patient
Doctor may recommend limitation of specific diet intakes. A notification informs doctor of selected diet recommendation based on the chronic condition selected above.
Other recommendation in the health plan includes:
- Smoking Cessation
- Programme Recommendation
- Active Ageing
Administrative details
After you have completed the health plan crafting, some administrative details needs to be verified such as:
- Doctor’s name & MCR
- Start & end date of health plan
- Recommended next visit date
- One thing to start working on
- Records of the health plan discussion with the patient.
Once all the details are filled in, press “save”
Vanda will save the current health plan as a draft for future editing. If doctor is ready, the submit button will appear after pressing save. Click on “Submit” to send the Health Plan to Healthier SG repository.
Health Plan List
Returning to the Health Plan List, Doctors may:
- Track the health plan submission status
- Preview or printout/save a pdf of the health plan for the patient
